- M.P. domicile/Mool Niwasi certificate
- Recent passport-size photograph

- NAPS Apprentice Registration printout with Registration Number
- Printout of submitted application
- Class 10th (Dasvin) certificate in support of Date of Birth and educational qualification
- ITI pass certificate and all semester/year mark sheets (NCVT/SCVT) in the relevant trade
- Caste certificate in prescribed Government of India / M.P. Government format for SC/ST candidates
- OBC (Non-Creamy Layer) Certificate in prescribed format, valid and current (issued recently); OBC Caste Name must appear in Central/State Government list
- M.P. Mool Niwasi (domicile) certificate issued by competent authority
- Disability Certificate (40% or more) from competent authority / District Medical Board for Divyang/PwBD candidates
- Medical Fitness Certificate from District Medical Officer (CMO)
